Our Clinic Policies
Leashes and Pet Carriers
It is required that all dogs entering the clinic be on a leash and be properly controlled. It is also a requirement that cats be on a leash or in a carrier. Please let us know if you require any assistance with your pet.
Appointments are required but every effort will be made to accommodate emergencies during business hours.
Pets undergoing procedures or surgery must have evidence of a current Rabies and Distemper Vaccine. Canine patients must have documentation of a current Heartworm test. An examination at Countryside Animal Clinic is required before any procedure or surgery can be scheduled.
Payment for Services
Payment is required at the time of service. We accept cash, Visa, Mastercard, and Discover cards.
Once you have established care with Countryside Animal Clinic, as a service to you, we will mail reminder cards to remind you of upcoming vaccines or preventative health care that is due. Because we cannot ensure that these cards will reach you each and every time, please keep a record of the due dates of your pet’s vaccines.
So that we can keep your records current and ensure that your reminder cards reach you, please advise us of any address or phone changes since your last visit.
